Welcome to our first survey on Bob Weir's solo efforts. We need the responses before September 20th so we can include them in DeadBase XI. You may want to read this over first and take a few days to jot down notes before you fill out the survey.

We will give a free copy of DeadBase XI to a randomly selected respondent for every 50 responses we receive. To be eligible for the drawing, you must respond to at least half the questions. (We have received a couple of completely blank questionnaires -- why should they have the same chance as those of you who invest time and care in your answers?) Your chance of winning may be effected by the number and quality of your answers. Winners will be notified by e-mail.

There will be many more surveys and opportunities to win free books, please do not submit multiple entries for any one version. We are requiring e-mail addresses to validate all responses (yes, we know many of you have more than one address so we ask that you exercise restraint).

Please note that this survey does not display correctly on all browsers. In particular, we have had problems with it on Netscape Communicator 4.61 for the Macintosh. That browser was only displaying five of the ten radio buttons which should be visible for the Jerry's Bands and Influences sections.
